At headspace Swan Hill, we capture data to help us provide the best care to young people. A hAPI survey helps us check in with how you are doing before your session so we can support you appropriately during your session.

When a young person has a scheduled appointment with headspace Swan Hill, they will be sent a text message with their hAPI survey along with a reminder message for the appointment time. This will also be sent the day before your scheduled appointment.

We ask that everyone that connects with headspace completes a survey before each session.

There are two ways to login and complete your hAPI. You can either create an online headspace Account and link this to your hAPI profile, or use your name and date of birth to sign in.

If you aren’t able to complete the survey before your session, we will provide you with a device to complete your hAPI when you arrive.

Follow the below link to fill out your survey.

https://hapi.headspace.org.au/login/young_person

Alongside informing your clinician how you are before your session, hAPI surveys also support us in understanding what we’re doing well and what we can improve on, so we appreciate you taking the time to read the questions and answer honestly.
